After your telehealth appointment: Prescription and/or Diagnostic Test referral form

Diagnostic Test (Referral form)

If your specialist has advised you during your consultation that your next appointment will be a review with a diagnostic report (such as a blood test), or requested you to have a radiology appointment (such as an Xray or CT scan) - Please allow 48hours for this referral to be sent to your email inbox (be sure to check your junk/spam folder). If you do not have an email, your form will be sent to your referring GP clinic who may be able to arrange a copy for you.

If you/your GP clinic do not receive this referral form, please reach out to Telecare by phone, SMS or online chat. We'd be happy to follow this up for you!

We'd prefer to have your form written by your specialist so that we can receive this report as soon as you've had your test, ready for your next appointment.


Scripts

We will soon be moving toward e-scripts! For now, a prescription can be sent directly to your nominated pharmacy. Please update these details on your patient information form, or call the team to provide this over the phone. Please call your pharmcy prior to your visit to ensure a prescription is ready for pick up.
