New referrers can start referring to Telecare using the below options:

Our Website: Fill out our patient referral form via our website: www.telecare.com.au/gps-contact

Email: admin@telecare.com.au (please attach your referral)

Phone: (03) 9052 4872

Efax: 03 8677 9399 (area code required for all states)

Medical-Objects/Healthlink: Telecare
